our process

step 1: choose a caregiver

An individual selects someone they already know and trust to become their caregiver.

step 2: complete onboarding

The caregiver completes the onboarding process, including training and required documentation.

step 3: begin care

Once approved, caregiving begins with ongoing support from the giv.care team.

Home and Community-Based Services (HCBS)

The HCBS Supported Living Services (SLS) Waiver helps eligible individuals receive care while continuing to live at home and remain active in their communities.

Colorado's Medicaid program, Health First Colorado, may provide additional support through programs like Community First Choice (CFC), offering expanded personal care, assistive technology, and remote support services for eligible members.

If you're new to Colorado's disability support system, your Case Manager can help determine eligibility, coordinate services, and guide you through the process.
